IMM 5669 Declaration Form requested

Hi

My application is in progress. Passed medicals July 24th and submitted RCMP background check. Today they requested I sign a new background form. In particular they asked

Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. This must be received at this office by August 23.

Has anyone been asked this before?

They either miss the initial form or they just want to confirm the information. It's not a big deal. Just fill it in and send it to them.
