I would like to seek help. Should I include IMM 5604 in my permanency application for my 3 kids since their father did not show up for them for 5 years now? My kids has been taken cared of by my parents. We're not married. Thanks.
 
IMM5406 u meant i think.

if you are not married and not in a partnership, you practically don't have any relationship with that person.
But u have to fill out the section for your children.
How old are they?

Yes, you have to.
On the form itself it says:

This form must be:
• filled out by the non-accompanying parent/guardian, former spouse or former common-law partner and;
• witnessed by a notary public.

I'm confused because my kids' dad has been absent/no show (no support or anything).

He's still their father, so he has to sign. In situations like that it's alwasy a good idea
to talk to a lawyer aswell.
