IMM 5538B (PR Voluntary Relinquish form)

One of my family member is currently doing the paper work on relinquish PR status due to not meeting the obligation (days of staying),
however he stuck on the IMM 5538B form...

The first part asks "I ____ of ____ solemnly declare that ..." I know you write your full name after "I", but what do you fill in after the "of" part?
Secondly, the form asks for a Deponent, can that be anyone?

of "the city and province/country where living. Deponent is the person making the statement.
