IMM 5532 Question 4 and 5

Hi,
I am reviewing all my forms using the Complete guide.
However, when it comes to Part B of IMM 5532, the Complete Guide (both in French and in English) mentions Questions 4, 5 and 6.... while the form itself end at Question 4 for Part B.

Am I missing something here? My best guess is that they updated the form, but not the guide... I just don't want to make a mistake!

Now I don't know how to delete this thread... it seems all the following question numbers are off in the Complete Guide.
