Hello guys,

I'm in need of help with IMM 5532 Part C Question #4.

In the basic guideline, it states that: When filling in the IMM 5532 – Relationship Information and Sponsorship Evaluation form, make sure you complete all of the sections on this form.

I have been living with my wife together but we also have visited each other during our relationship as well. I checked "Yes" to the Question #3 " Are you living together now?". However, Question #4 asks "If you are not living together, have you visited each other during your relationship?" and I wonder whether I should skip the question and leave "Yes" or "No" checkbox blank? or check "Yes" and explain the details?

Also the new document checklist states that "You must attach this checklist (both Part A and Part B, all pages) as the covering page for your application package" and the guideline states:

The order of documents should be as follows:
  • checklist;
  • open work permit application (and supporting documents), if applicable;
  • any barcode pages;
  • supporting documents, in the order they’re listed on the checklist.
So should the order be, Checklist, Open Work Permit, Checklist, Barcode pages, Checklist, Supporting documents? It seems very odd to do so. I'm very confused!

If you have any ideas, please help!

I just remember I answered this blank and didn't put N/A. Pretty confusing form. They should have put proceed to Question 5 for #3 if yes.

Now, as in Right Now, the date the form was signed. The assumption is that if you are living together Now, then at some point in the past you would also have visited each other and made the decision to live together prior to the date the form is signed.

So, if you are not living together Now, the next question asks if you have ever visited each other and when. That's all.

That makes sense too. But, leaving it blank I guess is fine too?

What do you mean by "Checklist, Open Work Permit, Checklist, Barcode pages, Checklist, Supporting documents?" Are you making multiple copies of the checklist? Or putting these documents in between the pages? Keep the whole checklist together on top, then do OWP, then Barcode forms, then supporting documents.

I left this blank, as I assumed it was understood that if I checked "Yes" to the first question, the next question is only for those who check "No". I think with this form since it's not a barcode one, leaving it blank isn't nearly as bad because it will be a person actually looking through your answers, not just the computer.
