IMM 5490 Are you in a common law relationship? yes provide evidence of your relationship. (for example insurance..)

Do I have to write like list with everything on a separate sheet of paper or in that box, what we have or do I need just to attach all the paper and the picture proof?

We only have joint rent and saving account together. Went on trips to Mexico and Ski trips. Movie tickets???

What else do I have to include? or any evidence like same address, family events, like the hole evidence? Photos Communication and other documents?

They also want written proof there, so how I did these types of questions was to write "please see attached evidence" in the box, then labeled the proof with a cover sheet and wrote out a list of your evidence along with a short "essay" pertaining to the (in your case) common-law relationship and why it is legit, then attach all your proof.
 
I have a question that kind of pertains to this...

Talking about relationship proof, it seems that sometimes the questions/proof/evidence overlaps.. you know? I'll try to explain better. Sometimes for a specific question, I will say "see attached proof" or something like that. But another question or section of a different form might need the same proof I've already used for another question.. Do I just make another copy and attach the same proof?

I was thinking of doing a whole separate envelope for our Relationship Evidence stuff, but some of that can also be attached to forms as additional info.. Do I just duplicate?

I hope I'm making sense.

Just put all of your evidence together in a separate folder/envelope and answer the same way to any and all questions asking about evidence of the relationship. Just say "Please see evidence of relationship", or if the question is asking for something specific, say "Please see Appendix A (for example)". This would be where you have created a separated sheet to explain something that can be used for multiple questions. Make sure to write your full name, UCI number (if you have one), DOB, sponsor's name, etc., and which question(s) this is to be used for, across the top of anything that you create. Don't simply says "See attached", because it's likely that the sheet will become `un-attached' when CIC tears through the application; they may not know which question the paper belongs to.

There's no need to make duplicate copies.

Random questions.. For all the letters that we submitting (one from sponsor, applicant, witnesses etc..) do they need to be in the normal "letter" format. With recipient and sender mailing addresses at the top and all that jazz? Or since they are not actually being mailed it's not necessary..

I was thinking of just putting all the info you just mentioned: applicant name, DOB, sponsor +date
 
If you're referring to the required Statutory Declaration letters for a Common-Law sponsorship (need at least 2), they should be written by the friend/family member in their own words. It should be a testimonial, of sorts, where they describe your relationship from their perspective. They should also include the fact that they know that you are living together (if you are), because some couples may have already established the required cohabiting period, but are living apart now (for various reasons). Basically, they need to corroborate the `story' of your relationship.

One they have written the letter (which you will probably want to see), they take it to someone that can notarize it for them/you.

For letters that the applicant and sponsor write, describing their relationship, it should be the ultimate love letter (censored, perhaps;)) that explains your relationship to the total stranger at CIC that will be reading it someday.

No need for sender mailing address and `all that jazz'; just tell the story.

Since you're on a roll, I have another question!

Since my partner has already dealt with CIC for his TRV (Multiple Entry) I know he must have a UCI# already. It should be on his visa that is glued in his passport correct? The "unaccounted" data that's on there is a 8 digit number, on the last line in the right hand corner... Is that it?
