Do i need to fill IMM 5409 (Statutory Declaration of Common-Law Union) for my wife. Its says for common law partner. Does this include spouse ?

So if i'm a citizen and am sponsoring my common law partner, my partner would need to fill in IMM 5409 form right? May I seek some help to clarify the following?
May I know if it's the sponsor (me) who is supposed to fill it in? or should it be my partner who is applying?
Country means the country for which my common law partner is from right?
Declarant means the applicant which my common law partner right?
Conjugal relationship means the same as common law partner right? (aka living together and close for a period of time?)
