Hello folks,

I am filling out MM5406 Additional family information form and need your advise.

I am a candian citizen living in Canada and am sponsering my husband who lives in India.

1. In Section A - on the line with Relationship listed as "Applicant" my husband has to write his name in english and native language - correct?

2. Then does he also write my name on the line with Relationship listed as "Spouse"?

ALthough if it doesn´t ask you for his name in his native language writing there´s no need too

Hello Members,

Are all the names in the whole application e.g Section A, B, C of this application form to be filled in the native language.
Should attaching an extra sheet with only all the names of the principal applicant. spouse, parents, brother/sisters work?
