IMM 5257 F form- does both the primary applicant and common-law need this?

Hi, pls help. I am helping my parents to fill up the super visa forms. Do they both need the IMM 5257 F ? Or just the primary applicant? The primary applicant is my mother and her common-law partner will be accompnying her.
