I would send a "complete" form. UNLESS you can find an instruction that says "complete and submit only those pages relevant to your application".

There's no questions there regarding my country - Israel.
So I don't know exactly what to fill there..

You must print and fill out the entire form. In the case something does not apply to you (for example the section for Afghan nationals), you write NA.

But as far as I can see there is plenty of information you have fill out. If it doesn't specify a certain nationality, then it means it applies to all nationals from Middle East and Central Asia (including Israel).
