Please, clear your situation, because:
1. The form IMM1017 is not an undergo form. It is being sent by CIC to the applicant only AFTER you applied for visa and if they require you to pass medical exam. This form you should bring with you to the doctor when you going to pass the exam.
2. As you were saying, you already passed medical test, that means that you were given a special form (confirmation), that u passed the exam. If the doctor works in e-medical system, they gave you an information sheet printout. If the doctor work with paper system, they gave you a copy of the IMM1017B Upfront medical report. At this point you already should have it, because you need to attach this paper to your visa application.
