Hi all. Thanks to this forum I was able to gain tons of help in applying for my wife's permanent visa to Canada. I finally got the package sent in and we've finally received notification that the office has received our application. The only matter left to us is the medical check-up for my wife. My wife was told by the hospital she contacted in charge of these check-ups that she needs to bring the (IMM 1017) form herself (something along the lines of "the visa office will give it to you). However I read online that the panel physician supplies it for you.

This is hopefully my last problem in a long and arduous process. Any help and advice would be greatly appreciated.
 
If you are applying outland, then you need to be firm with your physician's office that you are doing an upfront medical exam.

You don't need to provide any forms. They will give you a sheet with your information and a number on it which you will submit with your application.

If you re-do the medical because it will expire, on the instructions of the VO, the VO would send you the form. Didn't happen to us, but I believe because you already have a file open with CIC, the form would have the UCI or file number on it.

If this is the first time upfront medical, then you would not have nothing to give.

If the clinic is linked to the electronic system, then after the medical a printout comes out with summary information (name, photo, passport #, etc.) which should be submit together with the application package. If the clinic is not linked electronically, you are supposed to get a hand-written copy equivalent to the printout, which has information allowing CIC to link up the medical test to your application package.

Yes, be firm and tell them. Some receptionists/nurses do not have experience handling this, and may mislead you because he/she does not know.
