IMM 1017 Form - Doctors in British Virgin Islands

I'm trying to finalize all the paperwork to move my family back to Canada but I cannot seem to locate the necessary medical form so my wife can complete her medical and British Virgin Islands does not show up in the list of countries where they recommend doctors.
Can anyone point me to where I can print of the form? I assume we can't just wait till we move to Canada? (I and our two children are Canadians and my wife is Japanese who was a previous resident - but not finalized as "Permanent")

She could enter Canada as a visitor, since Japanese people do not need a visa for that. She could then get her medical done at a doctor in Canada. She would still have to get the medical done at one of the doctors CIC lists. It could be there are no panel physicians where you are currently living.
