I just did my medical yesterday and never got this. I asked 3 different times if there was anything I get back and they said no. All I recieved was a note from the doctor saying that I came in on that date and got the medical done. Is the form mailed to you then you put it with the app?

OK, you will not get the medical report. The doctor is correct. He will send it to CIC directly.

On the Section 2: Immigrant's Guide pg 6 on the medical section it says:
The DMP will give you Copy 2 of the Medical Report: IMM 1017 EFC - Medical Report Section A. Include the original of Copy 2 with your application. (Photocopies will not be accepted.)

It also says this on the US country specific checklist. Just not sure if it's mailed to us, then we send it in or if we need to go back to the doctor's office to get it.
