IME number for proof of medical exam for PR application via express entry

Hello all,

i received an ita for my express entry profile (cec) and have some confusion regarding medical exam proof. I had gotten my medical exam done in mid 2019 for my study permit application, but I am unable to find the IME number associated with it now.
1. Shall I upload a blank document in the medical field?
2. Shall I upload a LOE, telling them my old study permit number instead?
3. Shall I get an upfront medical exam done?

Your old medical exam will no longer be valid (validity is usually one year), so you may as well get a new upfront one.
