I have a question about the STATUTORY DECLARATION. My mother is the principal applicant so I of course checked the principal applicant box. SO I checked the second box for spouse and dependent child aged 18 and over for my dad, sister and I. On the last page it has a line for signature. Should we put our signature? Or should it be signed by my mother?

Hi Tessa_13

Are you talking about Schedule A- Background/Declaration (IMM 5669)

I guess each one of you,

1. Father
2. You
3. Sister
4. and of course Your Mother (principal applicant)

Each have its own IMM 5669 completely filled out, signed, and included in the principal applicant's package.

Hello plussy.

Yes I'm talking about Schedule A. So we will sign the form our selves right? Because that is what we did. My mother send the application to us for us to sign it then we sent ti back to her in Canada (I'm in the Philippines).
