I have been asked to send "Current Bank Statement" along with the medical Test

I have been asked to send "Current Bank Statement" along with the medical Test Request on November 9. 2012.

Would appreciate help in following Regard:

1. How many months of bank statement should be sent at this time?
2. Who is the appropriate verifying authority of "Bangladesh Shonchoy Potro (BSP)" ? how should l make the documents ready to send?
3. In the requested "Personal Net Worth Statement" ..Where exactly should the BSPs should be shown?
4. I have a car loan ..which i can settle if necessary before my departure to canada..is it necessary to declare the loan in that case?

I am new to this Forum. So my warm wishes to all who are using this resource for mutual help.
