i did mistake in 5669 schedule a background / declaration

My wife submit my application on 21 sep 16. She got AOR. now we are waiting for SA. But i look imm 5669 i did a big mistake on it i want to change it. how is possible should i request to re send only imm 5669 or any online form to inform?

You can fill out the form again, then scan and send it in via Case Specific Enquiry (google that and it's the first CIC link). Use the application number from your wife's AOR.

i searched on google its show country where my file transfer... shall i wait for Sponsor approval. Because after SA its transfer to London Visa office.

Yes. At the end it allows you to attach files. I would do it sooner rather than later because they haven't spent a lot of time processing yet...
