i am marrried but my contract used my single surname

hello everyone,
hi everybody pls.help me i have big problem and got confused i AM just married last july 25 2013,thats why all my documents is still my surname when i was single,mY surname when i was single is Ms.Montemayor when my prospect employer ask me my whole name that is written to my passport i told her my whole name WITH my surname montemayor because it is on my passport name ,now when i am completing my documents needed in applying to canada when i received my lmo and contract it is still montemayor now i downloaded a form from the canada immigration what should i declare or check in marital status?do i check the married box?even i used the Montemayor my single surname?is it ok i just put my whole name when i was single just to follow what is written to my passport and contract then i just check the married box and submit a marriage contract as additional documents?how about my nbi clearance do i need to submit the Montemayor or Condeza surname ?because i get two nbi clearance my single surname and married surname so,which of them do i submit?do i still follow the montemayor to all my documents?did it affect my application on it?what should i do ?pls....help me
