How to write inquiry letter via fax or email to New York office?

Hi guys,

My application for work permit to New york office is almost 2 months and did not heard anything from
them. They already asked for my passport and i already send it last 2nd week of october. unfortunately
i lost my tracking number when i send my passport.

Can anyone help me how to write an email or letter to be send to new york office asking if they receive
my passport and asking for the status of my application? My employer really need me to start Asap.
