Can anyone tell me what is the format of writting an explanation letter for the changes and update made to previously submitted forms?
I am going to submit my Ssupporting docs to local CHC and there are some changes and updates.
Thanks in advance.
 
Keep it Simple.

In my case, I gave reference of Form and Question Number and than explain in one line what changes have been made from the previous form.
