You should get a "statement of account" from your bank[your savings/chequing acct and investments(if any)] - whatever the current balance is, write it in the box. Submit the proof as well.

Tell you a secret, we used to borrow from each other (Int'l Students) to make a healthy balance like $10,000. A personal assessment/bank statement isn't a compilation of 6 months' transaction rather a one-page document where it states all your assets and liabilities (like SenoritaBella said above). So, whatever comes in your assessment, round it up or put the same figure in the "funds available" section of your form (don't leave it blank). The assessment/letter from your bank is your proof of funds.

When apply online, they don't require CUSTODIANSHIP DECLARATION, why?

And how can I sign my APPLICATION TO CHANGE CONDITIONS, EXTEND MY STAY OR REMAIN IN CANADA AS A STUDENT?

Provide a specific number(same amount as on the proof you get from the bank) and attach the proof.

If you are not employed, then you can go with "not applicable" if that's an option.
