Google I-94 and look for the .gov link of US government, input your name and passport number- it will show your entry/exit records of the last 5 years, and only takes a few seconds

Do not expect a fast response. It is frustratingly slow to get "Information Regarding Entry and Exit" from them. I've done it... It took 10 weeks to get the data.
I don't know why but that site shows no record for me. For the past 5 years I was in the US several times and it used to show all entry-exit but several months ago it started showing no records. I emailed them but they just replied that if I want entry-exit info I need to make an application. It's weird.

If it helps, I specifically asked for "Entry and exit dates for the past six years, including Port of Entry/Exit".
