HOW TO NOTIFY CIC OF DESIGNATED MEDICAL DR ISSUES

Does anyone know how to do this?

I have one Dr who doesn't allow pts to cx appts over the weekend yet requires 48 hr notice for Mon/Tues pts, then wants to charge a bogus fee if they miss.

Another, I suspect, isn't using current Schedule A forms (it says it's from 05/1998) and I pray my photocopy is enough.
