Hi,
I'm living abroad for 10 years after expiration of my permanent resident card, as I couldn't find a job as a medical doctor in Canada.
Now I would like to get back and live in Canada and practice a non-medical activity.
As I have never officially declined my permanent resident status (though I did not fulfill my residency obligations), is there a way I could still go back and stay in Canada?

You would need to fly to the US and try to re-enter Canada through a land border (using your COPR and traveling in a private vehicle) without being reported for failing to meet the residency requirement. If you are reported, you'll be called to a hearing and should expect to officially lose your PR status (and be ordered to leave Canada). If you manage to enter without being reported, you'll need to live in Canada for two years straight without leaving before you'll be able to renew your PR card.

Since you've been away for 10 years, there's a high chance you'll be reported when you enter Canada. However you can certainly try.
