How to fill Family Member Representative Form 5713E

Hi guys,

Please help me to fill in Form 5713E. I use myCIC account to apply for my Mom and Dad, but I dont apply for TRV along with them ( I've got TRV already). What Name should be filled in the Section A? My name or my Mom's name because she is applicant and my dad is her family member.

Under Section A:
1. Family member representative (Either your mom or dad).
2. Date of Birth of the Family member representative.
3. Name of the family members (do not include the Family member representative name here).

Say I guess your mom is the Primary applicant, then:
1. Family member representative: Your mom's name.
2. Date of Birth: Your mon's DoB.
3. Name of the family members: Your dad's name.

Under Section B:
Signature of your dad & date.

Under Section C:
Signature of your mom & date.

who is the one will fill up in section A? and section B? and C? please help me.
