How to apply for child visa long with spouse

My spouse will be travelling from India to Canada. I am creating applications for both my spouse and my child.
Does my child have to be apply using a representative? Or I can create an application just for my child

I am stuck in choosing which visit to select , Visit-tourism or Visit other. my baby is 4 months old.
also in IMM5257 , I don't know what choose, tourist or visit ? can you please advise
