While applying online for a visitors visa, after filling out the IMM5645, just leave all three fields where one is required to sign blank? Is the date to be added or kept blank as well?

I have the same question. Looks like IMM5257 can be signed online. I'm wondering about IMM5645 or other forms. I'm using my account to apply visas for my parents. Do they need to sign the last page? Will be really appreciated if someone help me in the question
