Help with Voluntary Relinquishment of Permanent Resident Status

My dad is outside Canada right now. His PR card is expired, he decides to relinquishment his permanent resident status. He recives an email along with the IMM5538B Voluntary Relinquishment of Permanent Resident Status form. However, we don't know how to fill it in. Please help us with it.

Which part are you not clear about? The first half he fills out. The second half from 'deponent' is done at the visa post. They will call him in for this. For example if he was in India he would fill out the form as:

Country - India, Province (State) - Punjab.

In the matter of IPRA .....I First Name, Middle Name, Last Name of 100, Punjab High Street, Punjab, India solemnly declare that I relinquish my Permanent Resident status of Canada obtained on date X with effect from date Y as signed below. I do this voluntarily and being fully aware of the consequences. I fully waive all appeal rights in relation to this decision (you have 30 days to change your mind if you don't waive appeal rights).
