Help! Which checklist should I follow and use? (Applying in the Philippines)

The country specific requirements doesn't include the completion of the Family Information (IMM 5645) form. Instead, a "Personal Information form" is available for download in philippines.gc.ca. There is also a difference in the photos that need to be submitted. I'm getting confused... Please help me decide...
