we are done with out file and we still have form 5562, do we have to fill this form too????

We did not submit a 5562 with our 'outland' application. Is it on your Document Checklist? Is supplementary travel even applicable to you (the applicant)?

It's in "western europe" guideline. So if you are applying through one of the Western Europe VO, it's in your guideline, but only applicable to Paris VO. So if you're applying through Paris, then you need to complete the form.
If you're applying through one of the others VO (London etc), then no need - don't worry about it.
