Help Needed !

Hi All,

I will eligible to apply for citizen by mid month of october.

can anyone let me know if we need to send these two photocopies for citizen application ?
1) Notice of assessment
2) Copy of In /out Entries Stamps on Passport Page

3) Do we need to send any additional document apart from these :-

- photocopy of pr card
- photocopy of driving license
- photocopy of passport first and last page
- two photos

4) do I need to request for Police cleareance certificate from canada ?

Here are number of days I lived in canada.

2012 - 179
2013 - 365
2014 - 316
2015 - 328
2016 - 273

5) secondly I dont have stamp of entry to canada when I came second time . does it matter ?

For #5 you must have scanned your pr card at the automatic kiosk at the airport. If yes, your entry will be recorded electronically.

I checked today , they stamped on my custom check form rather than my passport. But I have only picture of that form.
