So you all take the medical and wait for whether you are inadmissable. Then you hire a Canadian lawyer to work with you and your medical team to outline current and future medical costs to show that it will not exceed current limits. IRCC doesn't factor in if you can get medicine or treatment in your country. They will look at the current and long term prognosis and any future treatment needed (surgeries, therapies, medicine etc.).
Got it and so grateful for your help, Naturgrl!

In this situation, declaring myself as non-accompanying seems doesn't help at all. Seems we just do nothing, accept the health check, and reply to PFL if it is decided as inadmissible.
My wife strongly against me staying alone in our home country...

Declaring yourself as non-accompanying could also get your application denied for misrepresentation. Also in most cases would not let you remain in Canada with your family other than as a visitor so would prevent access to healthcare or at the very least would require you to pay for care. You can’t apply for an SOWP if your spouse has a BOWP for example all while declaring yourself as non-accompanying. They contradict each other.

Yeah, this seems the most feasible approach. Thanks so much for all the explanation and help!!

Got it and it's pretty clear to me know. Thanks so much for all the help!

Would suggest you do some brief calculations on what you think your costs will be (you can ask for billing records for your provincial healthcare plan) to see how close you may be to inadmissibility.

I also wondering how could I have an evaluation of this in Canada. Because currently, all my treatment is outside Canada. Looking into this...
