help! IMM 1344 form

just confuse on question 2 on Sponsor personal details.. Have you ever used any other name(eg, Nickname, maiden name, alias, etc) do i have to mark yes if i used already my spouse family name? but not yet updated my PR card as well as my passport only my drvers licence and health insurance.. thanx

you would mark 'yes', and indicate both your maiden name and your married name.
