Dear xxx:
This is in reference to your application for permanent residence in Canada. In order for us to continue the processing of your application, we require the following documents:


Evidence that your spouse has undergone a medical examination as requested. This must be received by this office by September 28, 2012.
Please note:
All documents must be accompanied by English or French translations. When original documents are requested, notarised documents are not acceptable.

NOTES:

my wife has done the medical exam end of December 2011, we also send the document to our lawer, not
even one year since then, how come they ask her to do the medical exam again, i wondering maybe my lawer has not sent the ME exam document to CIC yet? I am confused. what do you think, guys.
 
I would suggest you get in contact with your lawyer to find out if he/she sent the form to CIC.
They aren't asking your spouse to go through another medical exam just provide proof that she has done one. Go to your lawyer first and see what's been done with the form...
 
I would suggest to ask your DMP, whether he sends the medical or not?

Only the physician who performed the medical exam can send the results to CIC. No applicant, lawyer or other representative can do so.
