I will fill out additional family information form as an applicant but does my wife need to fill out too? if yes then what she need to write in first four question?

applicant: when she will fill out this form what she need to write here? her name or my name?

your spouse:

your mother: when my wife fill out this detail,whose mother name should she write,her or mine?

your father:when my wife fill out this detail,whose father name should she write,her or mine?

You are principal Applicant in your form and You wife will be applicant in her form.So in your wife's form, she will fill as Applicant and all details would be related to her as Applicant.
