i'm wondering, when an application has been approved by ircc officer and submitted one's passport can the application still be denied or sent back with no stamp on it ?

Okay, when the ircc officer is about to stamp a visa on your passport doesn't he/she first re review your application once again, or when you're approved it means that your application is closed(concluded) and won't be checked again ?
