Sorry I wrote that wrong my wife is the only one being sponsored her son is 21 yo and will stay in china so I am assuming he does not need a health assessment. Correct?

Only your wife and her dependent children would need a medical. Her son who is 21 yo is not a dependent child any more so he will not need one.
