I called today to set up an appt to get my exam for my application (common law in Canada). The woman on the phone told me I was able to pay the $500ish fee for the application, use that receipt to get a health card, and then use that card to save some money on the exam(not to mention have a health card). But don't I need to take the exam just to file for PR? Or are the submitted seperatly and it's ok to file first and then get the exam after as long is it's not too long after? Say within the week?

You need to send proof with your application of having done your medical exam. See your document checklist.
 
Another question regarding the police certificate.

Does that need to be sent in with the initial application? I've seen it say it takes anywhere from 8-20+ weeks for a response, which would severely push back when I can apply.

Or can I apply and send in the certificate when I get it?
 
If you send an incomplete application, it can be returned to you before even sponsorship has been approved. But I have also read several posts from people dealing with Buffalo that they included proof that requests for the FBI clearance had been submitted and a letter explaining that due to the long processing time they would forward the clearances when received.

I personally would not recommend sending an incomplete application. If you want to solicit more advice on the subject, I suggest you post a thread in the Family Class folder. It is a very active folder, and I am sure you will get some great advice.
