Well first of all congratulations on getting your PR. I believe you take your PR card to the nearest Service Canada office and get your health card.
 
Yes you can, just bring confirmation of permanent resident, passport and the old health card to licence and registry where your residing.

Depends on where you are. I'm in BC and was told:

1. Make a copy of your CoPR letter, write your personal health number at the top of it, and mail it to them.
2. You should get a letter back from them confirming they received it and updated your information, I was told it takes about 4 weeks.
3. Once you get your pr card in the mail, then take your card and MSP letter to the nearest office that issue driver licenses and they will then take your photo and give you a new card.

I was told don't go in until I got the actual pr card. This was the same info several of my coworkers were given as well when they called in and asked about it.
 
Can go into Service Ontario with your COPR and they'll give you a new OHIP card.
