I already have the notarized affidavit that both names represent one and the same person!
 
No, middle names aren't always declared in day to day stuff.

I have a middle name and most jobs don't mention it, but it's on my passport and degree.

It only means if you had a maiden name or you changed your name.
 
I hadn't even considered an avidavit for my middle name :p
