I am known by First Name, Middle Name, Last Name in Government Issued Documents and by First Name and Last Name in Employer Documents.

Do I need to state yes to this question?

I already have the notarized affidavit that both names represent one and the same person!

No, middle names aren't always declared in day to day stuff.

I have a middle name and most jobs don't mention it, but it's on my passport and degree.

It only means if you had a maiden name or you changed your name.
