Your study permit will correspond to the date on your deferral letter. The embassy doesn't issue on the visa for the missed semester.

You can apply for visa extension while living there, it is not that difficult. What I have heard is that you get the visa for the duration of your course or your passport expiry date whatever is lesser and you can extend your study permit/visa or renew your passport while staying there.

Usually the status of your medical is updated within a week on cic portal if you have applied online.

I have not send my deferral letter to CIC
Will sending this deferral letter to CIC change my visa time from 2014-2018 into 2015-2019 or NOT? or would I only be able to get an extension after entering Canada?
And what If i get visa for 2014-2018, will I be able to enter after one year in september 2015?
 
you can easily apply for extension of your study permit/TRV once you are there. But as bfarooqui mentioned earlier that they would asked for updated acceptance letter, in that case, you would have to send them. If you do send your deferral letter by yourself or on their request, you would ultimately have to send it for their record. I also have not sent yet.

Refer to my answer posted earlier. Be sure that the embassy DOES NOT issue the visa for the date already passed. They always ask for the deferral letter to issue the visa with the date corresponding to your revised admission letter/deferral letter. In gour case, the visa shall be issued for sept 2015. Hope that helps.
