Getting medicals in Toronto Canada while applying for EE from USA

I tried to find related threads but couldn't get all the answers.

I live in Philadelphia, PA, USA and was preparing to apply for Express Entry. According to the CRS calculator, it looks like I have 490 points.

I have almost everything compiled but had a few questions about medical exam.

1. Does anyone know how much it costs in Philadelphia at this location?

2. Is it possible to visit Canada on visitors visa (which I already have) while your EE application is pending with CIC?

3. Has anyone gotten fingerprints for FBI PCC at any Philadelphia Police Station? How much did it cost?

4. Is it possible to get medical exam done while visiting Canada since I'm pretty sure from what I have read so far is much more cheaper ($200-$250) than North East USA ($450-$550)

5. Is it possible to get H1B visa extension stamped in Canada (if I get approval) while your EE application is pending with CIC?
I have H1B visa until 10th June 2018 and have applied for extension, currently waiting on the final decision.

Do you know if upfront medical exam can be done here? Also, do we need to get medical exam done for kids too, if yes is the cost same? Mine is 4 years old. Any help on this is really appreciated.

Upfront medical exam can be done in any part of the world and yes it's for kids also, cost differs per physician.

Just call and ask them. Generally they charge less for children.

Yes.They do upfront medical exam! You can call them and ask about charges for children's medical exam fee but for adults it was $423 at Philadelphia location. They do charge a portion of fee ($65) when you book the appointment.
