Getting a Status Verification Letter from IRCC

I am currently trying to add a middle name to my legal name via Service Ontario, they have requested from me a Status Verification Letter from IRCC.

Would that be this?

And for Part B, I choose "Verification of Status" for Part B1

For B2, it is asking me for Immigration Document Date of Issue, what do I fill in here? do I Leave Blank?
For B3 it is asking for Canadian Port of Entry, do I Leave Blank?
For B4, I choose Immigration Visa and Record of Landing - Confirmation of Permanent Residence, is this correct?

For Part B5, I put Other and put this description:

I have never had nor have I applied for a Verification of Status before.
I am trying to add a middle name to my Legal Name via the Office of the Registrair General in Ontario. And they have asked me for a "Status Verification Letter from IRCC". So I need a letter from IRCC about my current status in Canada, I have been Canadian Citizen since 1997.
What I need please is a letter from IRCC stating my current status in Canada (which I am a canadian citizen) so I can send a copy to the Office of the Registrair General.

And for Part C, it says Did you enter Canada as visitor, student, worker, PR, Protected Person, none of these apply to me? As we came to Canada as landed immigratns back in 1992 (i was 2 weeks old lol), then we became citizens in 1997

P.S I do have UCI with IRCC, is there any online portal where I can just login to get this? Cuz I sponsored my wife a few years ago to come into canada.

You came as a PR.

You can find on any if your immigration docs like a citizenship certificate or a citizenship card.

1- Really, if we came in 1992 as Landed Immigrant, it counts as PR?

2- I already have UCI, I was asking is there a way to get this info by using this online to get status verficaiton letter.
