Generic Application form Need Help.

Hello

while filling up application form i got confused at these i will be very grateful if some one guide me.

In Application Details
Point 4: Immigration Office Requested for Processing this Application.
Point 7: Have u received your certificate de selection du Quebec(CSQ)

In Personal Details:
Point 3: UCI

National Identity Document: what does this means?

pls guide me.

Application Details Point 4 - specify "CPP Ottawa" as the immigration office.
Application Details Point 7 - answer "no" (I'm not sure if you have one, but this is CEC so you are not going to Quebec by definition)

Personal Details Point 3 - this is the Client ID mentioned on your work permit IMM 1442 or similar document.

National identity document - an identity document other than a passport issued by your country. If there is no such document (such as in my case) then select "no".
