Hello,

Please suggest what I need to fill in the section "Immigration Office requested for processing this application". The guide says:
Indicate the name of the Immigration Office which serves:
your country of nationality, or
the country in which you are residing, provided that you have been lawfully admitted to that country for at least one (1)
year.
Note: Do not select the Centralized Intake Office (CIO) as the immigration office requested to process your application. Although you may be required to submit your application to the CIO, you must indicate the name of an appropriate visa office using the criteria above.

I have been in Canada for over an year and would need to go back to my home country in 2/3months from now as my work permit is expiring.

Do you plan to stay in Canada based on your AOR and subsequent BOWP? If yes, put CPC-O. If no, then use the VO location for your home country.
 
OK, Thanks... Also there are quite a few places in the form which asks the 'from' and 'to' dates. I entered the from date as my landing date in Canada... how about the to date ? do I enter today's date ? or the date when my work permit expires ?

Best option is to say 'Till date' or 'Present' or 'ToDate'....

The date you are going to be submitting the application.
