GCMS notes, no email request for information release form?

I sent a request for my GCMS notes on August 3rd, but had forgotten to include the information release form. I haven't received an email asking for this form yet. I haven't bothered submitting a new request assuming they would ask for the missing document. So far nothing, should I continue to wait, or go ahead and do another request with the document included this time?

I did that last November. If you received the confirmation email right after sending the request there is a reference number and a email address in the correspondence. Fill out the release form, attach it, use the reference number with an explanation. It worked for me
