Gaps Between new and old passport

I have a minor gap of 20 days between my old and new passport, any idea what should I write in clarification?
also in question 11 of new updates form, it ask where were you physically located? is it going to be the employers address or will it be your home addresses during you worked for that employer?

What was the reason for the gap?
For question 11 it would be where your employer is located.
