i need your advice regarding the receipt of RPRF. My brother paid my fee through his credit card. Now my question is what I have to write in address section and phone number. Do i have to write my address n phone number or my brother's?

It should be the main applicants details only, not the person who paid. The name of the person who made the payment will show up though.

Not Correct,

the Name of the person who has paid will be on the Receipt (kindly note that a receipt is always issued in the name of the person paying and not in someone else name.)

you have to fill in your details in the space given for the address etc.

Exactly, visa is valid for One year from the date of medical or expiry of passport, whichever is earlier.

If Principal applicant and dependent had different date of medical, date of earlier medical is considered.

For passport, Principal or dependent whose passport expires earlier is considered.

1. My passport expiry month is Aug, 2016. can i enter in Feb, 2016 without renewing it.? i Can do the renewal from Indian Embasssy in canada right?
2. how about the stamping. it will be in my old passport correct? do i need to carry both of them all the time?

Hi, my cousin got Medical Request. He had spinal surgery in 2011 and since then he is fine and working full time. Does he need to disclose about surgery even if doctor does not ask about it? Does the doctor ask about the major surgeries in the past? Would it have negative affect on the medical?

As per my experience, Doctor will ask about your complete health history. so dont worry tell them everything what ever they ask.
Take any proofs that you have?
