Congrats mate !!! any Doctor certified by CIC who can give you the desired reports within given timeframes as soon as possible is good for you.
Dont go by his or her degree and experience. These tests are carried out by lab technicians and reports are generated.. So getting an appointment of a good doctor is not relevant here ;D ;D

You're welcome. There are specific tests in every country's package. You just have to say that you want an appointment for medical exam for Canada and they'll give you the details and time slot.

You need to carry your original passport and the Immigration identification number with a printout. They generally are aware of what all tests need to carryout for Canada immigration.

You're welcome. I don't think its possible because there is an IME number mentioned on the form that needs to be provided by CIC.
